【摘要】:ManyU.S.healthcareandhealthinformationsystemsleadersbelievethatelectronichealthinformationsystemsthatincludecomputer-basedpatientrecordswillimprovehealthcare.Toachievethisgoalthesesystemswillneedtoprotectindividualprivacyofpatientdata,provideappropriateaccess,anduseadequatedatasecuritymeasures.Soundinformationpoliciesandpracticesmustbeinplacepriortothewide-scaledeploymentofhealthinformationsystems.Strongenforceableprivacypoliciesmustshapethedevelopmentandimplementationofthesesystems.Thepurposesofpatientrecordsaretodocumentthecourseofthepatient''sillnessorhealthstatusduringeachencounterandepisodeofcare;tofurnishdocumentaryevidenceofthecourseofthepatient''shealthevaluation,treatmentandchangeincondition;todocumentanindividual''shealthstatus;toprovidedataforpreventivecare;todocumentcommunicationbetweenthepractitionerresponsibleforthepatient''scareandanyotherhealthcarepractitionerwhocontributestothepatient''scare;toassistinprotectingthelegalinterestofthepatient,thehealthcarefacilityandtheresponsiblepractitioner;toprovidecontinuityofcare;toprovidedatatosubstantiateinsuranceclaims;toprovideabasisforevaluatingtheadequacyandappropriatenessofcare;andtoprovidedataforuseincontinuingeducationandresearch.Healthinformationisabroadconcept.Itincludesallinformationrelatedtoanindividual''sphysicalandmentalhealth,theprovisionofhealthcaregenerally,andpaymentforhealthcare.Thepatientrecordisamajorcomponentofthehealthinformationsystem.Thecreationofelectronicdatabasesandcommunicationprotocolstotransferdatabetweensystemspresentsnewopportunitiestoimplementmoreeffectivesystemsforhealthinformation,toenhancepatientcare,reducethecostofhealthcare,andimprovepatientoutcomes.Nationalstandardsguideallthathaveresponsibilitiesforrecordsandinformationsystemscontainingpersonidentifiablehealthdataandinformation.Thisguidealsoacknowledgesthelargeandgrowinglistofhealthinformationdatabasesalreadyinexistence.Thesedatabaseshavebeenassembledtopayforservicesrendered(insurance),tovalidatetheappropriateuseofpatientservices(utilizationmanagement),tosupportpolicy(nationallevels),togatherdataforresearch/trackingofspecificproblems(registriessuchastumor,trauma,birthdefects,mentalhealthcasemanagement),topreventthespreadofdisease(requiredreportingofcommunicablediseasessuchastuberculosis,gonorrhea,AIDS),andtorespondtonewuseswhichareproposedeachyear.Nationalstandardsdelineatingprinciplesandpracticesintheareasofconfidentiality,privacy,access,anddatasecuritywillprovideaguideforpolicy,law,andsystemsdevelopmentandabaseforstandardsforelectronichealthinformationregardlessofitslocation.1.1Thisguidecoverstheprinciplesforconfidentiality,privacy,access,andsecurityofpersonidentifiablehealthinformation.Thefocusofthisstandardiscomputer-basedsystems;however,manyoftheprinciplesoutlinedinthisguidealsoapplytohealthinformationandpatientrecordsthatarenotinanelectronicformat.Basicprinciplesandethicalpracticesforhandlingconfidentiality,access,andsecurityofhealthinformationarecontainedinamyriadoffederalandstatelaws,rulesandregulations,andinethicalstatementsofprofessionalconduct.Thepurposeofthisguideistosynthesizeandaggregateintoacohesiveguidetheprinciplesthatunderpinthedevelopmentofmorespecificstandardsforhealthinformationandtosupportthedevelopmentofpoliciesandproceduresforelectronichealthrecordsystemsandhealthinformationsystems.
